Who can be a member of the Society?
- All active registered permanent or contract employees and doctors of Holy Family Hospital are fully eligible to become members of the Society.
How can one join the Society?
Submit the official membership application form along with the following mandatory self-attested documents:
- Valid ID & Address Proof: PAN Card, Voter ID Card, Aadhaar Card, etc.
- Photographs: 1 Latest Passport Size Photograph.
- Employment Proof: Joining Letter (Contractual or Permanent Appointment Letter issued by HFH).
